Physical therapy services
Our services include manual therapy, electrical stimulation, heat and cold therapy, ultrasound therapy, cold laser light therapy, spinal decompression, balance and coordination training, strengthening exercises and posture correction. Call the clinic to discuss an evaluation and an individualized treatment plan.
Your first visit
Bring your insurance cards and completed intake packet. Call the office to confirm coverage and any referral requirements before your visit. Intake packets are available in English and Spanish.

We accept Medicare, Medicaid and many commercial insurance plans. Call to verify your specific coverage and payment requirements.
Home-care physical therapy: $150 per visit, including the initial visit and follow-up visits.
Cancellation policy
If you need to cancel your appointment, call Crescent Physical Therapy P.C. that day. If you do not call to cancel your appointment that day, a $10.00 cancellation fee applies.
